A Breath of Fresh Air: Approaches to Doubling Lung Utilization

Purpose/Goal:

Currently only about 20% of deceased donors are lung donors. One of the primary determinants of lung acceptability is the PaO2/FiO2 (PF) ratio. However in a neurologically deceased donor this can be impacted by factors that are potentially reversible during donor management. This webinar will review strategies that can allow recruitment of the lung and thus an improved PF ratio, such as the use of ICU ventilatory techniques utilized in the management of ARDS patients. Together with the innovative use of ex-vivo lung perfusion and donors with hepatitis C we believe that donor lung utilization can be doubled to 40%.

Learning Objectives:

At the end of the webinar, participants should be able to:

  • Recognize the inflammatory and pathophysiological changes in the lung function of a neurologically deceased donor.
  • Identify an approach to treat all donors as potential lung donors by utilizing a structured approach to lung recruitment.
  • Develop a familiarity with the use of APRV (airway pressure release ventilation) for lung recruitment.
  • Discuss the impact of EVLP and lung donor research programs on lung transplantation.
